CHESTERTOWN — Choptank Community Health System received a $500,000 grant from The Harry and Jeanette Weinberg Foundation to support the expansion of its Chestertown Health Center, located at 126 Philosophers Terrace.
“This grant directly supported our mission to meet people where they are and remove barriers to care,” Choptank Health President and CEO Sara Rich said in a news release.
“We’re grateful for The Weinberg Foundation’s investment in rural health and its alignment with our vision for a healthier Eastern Shore,” Rich added.
Choptank Health provides care at the Chestertown and Rock Hall health centers through a mobile health unit, and has introduced school-based and community outreach services for residents of Kent County and northern Queen Anne’s County.
According to the news release, the Chestertown renovations enable Choptank Health to serve patients in upgraded medical and behavioral health spaces, including 12 exam rooms, multiple therapist offices and a dental wing with eight operatories.
Services at Choptank Health’s locations in Chestertown and Rock Hall include primary care, women’s health, pediatrics, behavioral health and medication-assisted treatment. The plan is to offer dental services in 2026, according to the news release.
Elvis Guzman, program officer at The Weinberg Foundation, said the foundation is committed to helping people achieve economic stability and well-being.
“We are proud to support this renovation, which will offer medical, dental and behavioral health services all under one roof, making it easier for residents in rural communities to access the care they need to thrive,” he said.
New medical patients are welcomed at all Choptank locations.
